I will email you upon receiving the form and will set up a consult if needed.

You agree to the following:

1. Age Policy: You are 18 years or older. 
2. COVID Policy: Please refrain from bringing any extra people. I will be wearing a mask during tattooing. Please stay vigilant at least a week before your appointment, and avoid risking exposure. If you can't, please do a couple of COVID19 antibody tests to prove that you're not sick. Keep an eye on the symptoms. 
3. Deposit & Cancellation Policy: You will have to send a deposit (current hourly rate) in order to secure your tattoo appointment. If you cancel your tattoo appointment less than a week before/I already sent you your finalized design or are a no-show, it's not refundable. The deposit will go towards your total payment. If I have to finish your tattoo in several sessions it will be applied at your last appointment.
4. You do not have any medical conditions contradicting getting a tattoo.  
5. Sending this form is not a guarantee of getting a tattoo appointment.
